New patients
Before your first visit
Most of what we need, we ask for on the phone or at the visit. This is what to expect.
Tell us where
The address of the visit, and the name of the resident. For a facility, the name of the contact person.
Tell us who
The resident's name and the person who makes their health decisions, if that is someone else.
Have the cards ready
Vision, medical and Medicare cards, so we can bill the right plan.
Fill in forms when you can
Our patient forms are online, or we bring paper copies. Ask for large print.
